WELCOME NEW PATIENTS! We at Pediatric Associates appreciate your decision to choose our practice to care for your child. We always welcome new patients. Our mission is to work diligently with you to promote a healthy environment for your child. We enjoy seeing and treating children from birth into college. We often continue to care for developmentally disabled or chronically ill children well into young adulthood. (Below you will find our Information Sheets that you usually have to fill out on your first visit to our office. For your convenience you can now print the form out at home, fill it out and bring it in on your first visit. The forms are .pdf files and require Adobe Acrobat Reader in order to print. If you do not have Adobe Acrobat Reader you can click on the link below to download your copy of Adobe Acrobat. You can download the forms in two ways: 1. You can click on the link and the demographic will load in your Internet Browser or 2. You can Right Click on the link and save the file to your computer and then open it with your Adobe Reader. When you come in on your first visit please present the Information Sheets you filled out at home to the receptionist at the front desk. Please Remember to Download and fill out ALL forms that apply if you choose to do this!)
